What do I get with this added service?

As a Medicare member, you get greater convenience, highly personalized attention, whole person support that addresses your life and health goals, and increased service coordination and guidance for you and your family to keep you living healthy and independent longer.

These broad-based services adapt to your changing needs, providing more intensive support as needed, and adjusting according to your wellbeing.

Services include:

  • 24/7 concierge service for questions, guidance, pre-procedure planning, and connection to reliable resources (including all North Memorial Health and Lifespark services)

Planning an elective surgery? Call us first – we’ll help coordinate a more seamless experience

  • 24/7 nurse practitioner/physician triage with easy-to-use telehealth and in-home visit capability service for urgent connections to avoid unnecessary hospital and ER visits
  • Proactive independence and life planning tools and support
  • Life Experience professionals, including Life Managers (registered nurses), who make in-home visits and coordinate with all other providers and family
  • Opportunity for in-home primary care services
What does this cost?

Under your Medicare plan, the usual co-pays and other fees apply for primary care visits (whether virtual, in your home, or in the clinic) and other services and procedures. Life care support and the concierge services are part of the value-added services provided through North Memorial Health and Lifespark at no additional cost under your Medicare plan.

What are some ways I can use this service?

There are many ways you can benefit from the Lifespark’s services. For example, if you are planning elective surgery, simply call the Lifespark concierge service and they will help you with planning and preparation for a more seamless experience. Looking for transportation options? We can help with that as well. Concerned about an older relative? We’ll answer your questions, provide tips, and connect you to the right resources. Not sure if you should head to the Urgent Care Center? We’ll talk through your options so you can make the best choice. Looking for planning tools to help you and your family stay independent and in control? Start with us.

How will this impact my current healthcare providers?

These services integrate seamlessly with all other services you are receiving whether from the North Memorial Health system or other providers. We integrate the life care services with your existing primary care provider and will work with any other providers involved in your care. The purpose of this service is to streamline and simplify care coordination across all of your healthcare providers to maintain consistency, convenience, and identify opportunities to create better health outcomes for you and lower costs of care.
